SHARING YOUR STORY CONSENT NOTICE

Latch Welsh Children’s Cancer Charity works with children and families as part of its activities. These include practical, emotional, and financial services to patients and their families, days out, events and holidays in Latch caravans, bereavement support and funding medical equipment and a range of clinical and research projects.

Please read the following consent form and discuss it with your family and then complete the form and return it to the Latch office or your Social Worker.

If you have any questions or concerns, please speak to a member of the Latch team.

Background

We sometimes use the stories and experiences of our families, children, young people, and supporters who have experience of cancer or leukemia in childhood to raise awareness and to demonstrate the impact of the support that we can offer at Latch.

These stories may be shared in our campaigning and fundraising materials, our website, on our social media platforms (which may be re-shared by third parties), internal communications, posters, leaflets, banners, videos, presentations, fundraising materials and any of our other publications in order to support our charitable objectives.

Depending on the consent given, the image/video may be used in press or media coverage to support our charitable objectives.

Our use of your story will be governed by this consent form and our Data Protection Privacy Notice. The privacy notice can be read here: www.Latchwales.org. Alternatively, please ask the Latch team for a hard copy.

Right to Withdraw your Consent

We will store your story securely for 5 years unless you withdraw your consent prior to this.

You have the right to withdraw your consent to us using your story at any time. This will not affect the lawfulness of any use of the story before you withdraw your consent.

Please contact fundraising@Latchwales.org should you wish to withdraw your consent.

By completing the form below, you are giving us (and any third party instructed by us) permission to use your story and to communicate it externally as indicated below.

I understand and consent to Latch Welsh Children's Cancer Charity using my story in all media including and not limited to: Latch newsletter, Latch fundraising materials, Latch digital and social media, Latch website, Printed media.

In consenting to the above use, I understand:

  • how my story and image will be stored at Latch
  • that if I withdraw consent for my story and image to be used or shared in the future, it may not be possible to remove or delete copies of it that have already been published or distributed
  • I do not own any copyright or any other rights of ownership in the story or image
